User Survey

I have been maintaining this site for over a decade. It provides data on state-by-state expenditures on dozens of categories related to aid to people with intellectual disabilities and their families, using a JavaScript library for drawing comparison charts of selected data. The foundation director asked for a user survey so they could better understand user interest in the site and cater their information appropriately.

The specifically wanted to avoid applications like Survey Monkey, preferring instead a home-grown, simple questionnaire that wrote results to a downloadable spreadsheet. We designed a simple, though long, form and invoked it the first time a visitor accessed the data charting feature, setting a cookie so they would not encounter it again. There was some JavaScript validation then a straightforward PHP script to write the form entries to a .csv file. A fairly commonplace little project, but important to their work.
